Health and Safety Waiver

At your first session, we ask that you sign our health and safety form below. Signing this form, you agree to: 

1. I understand that The University of Sheffield, Sport Sheffield Club activities may be physically demanding & that there will be a risk of injury when undertaking them.

2. In the event of an accident, injury, or damage to personal effects I acknowledge that The University of Sheffield, Sport Sheffield will not be held liable unless it's proven that it was caused by The University of Sheffield/Sport Sheffield’s negligence.

3. I certify that to the best of my knowledge, I do not have a medical condition, which might make me more likely to sustain an injury when taking part in Club Activities, nor am I participating against doctors’ advice. I understand to inform Club Officials if this situation changes.

4. I confirm that I will not undertake Club Activities under the adverse influence of drink or drugs.
